Erection disorders (erectile dysfunction), impotence
Every 5th men can be affected and at an age between 50 and 70 more than half of all men suffer from erection disorders. Not every man likes to deal with the diagnosis impotence (also called erectile dysfunction) although the diagnostic and treatment methods have improved considerably. Every man suffers during his life time from one or even more episodes of sexual failure or the fear of failure. But we talk about a pathological failure when sexual intercourse is not possible permanently due to an erection disorder for a period of more than 6 months. Although the press reports something different: with an increasing age the potency is decreasing but however still remains. The erection (stiffness of the penis) is interplay of different forces eg. fantasies, erotic and sensory stimuli and the stimulation of nerves, a muscle extension of the corpus cavernosum, increased blood flow and a venous drain blockage (Abb. 1). If the interaction between blood flow, the expansion of the corpus cavernosum and the blood discharge is disturbed, an undesirable limpness is the consequence. If different organic functions must interact, some disorders do not stay out.

A number of possible influences can lead to an erection disorder. There are organic factors as well as vessel amendments, nerve damages, diseases of the sexual organs or hormone disorders and also medications like vasodepressors and psychotropic drugs and also alcohol and nicotine have a negative effect on the men’s potency and the erection ability.

To give the factors an order the patient is given a questionnaire. By this cooperation between patient and doctor the burdening factors can be evaluated and classified. Together it is discussed the three big disorder circles where the reasons for the dysfunction can be sorted in (Abb. 2).
In only 5% of the cases the patients suffer from a hormone disorder which means the hormone deficiency can be evaluated by blood examination. 25% suffer from psychogenic and psychological or nervous problems and the rest suffers from damage to the vessel system. Depending on the reasons for the erectile dysfunction there is a broad treatment spectrum ranging from medication up to operation. A large part of men can cope with a temporary or permanent brace aid in form of pills and tablets. Others learn to give themselves injections (picture 3) or use a vacuum pump (picture 4). According to the reasons and the individual appraisal of the patients a high proportion can be treated satisfactory and to the fully satisfaction of the sexual partners. Basis for such a therapy is overcoming one’s inhibitions. The respective man must be prepared to talk about his problems in detail and must agree with describing his complaints to foreign people. It goes without saying that the medical staff is treating this information discretely and confidentially.
